Context
For many young people, turning 18 is a challenging phase, especially for those with experience in youth care, debt, mental health issues or a limited support network. The municipality of Hengelo noticed that many young people lost overview during this transition. Together with Score Agency they developed LUMOS, a digital future plan that helps young people take control of their lives.\n\nLUMOS strengthens conversations and provides structure. It is designed together with young people and feels like their own plan instead of a professional dossier.
Vulnerable young people lacked ownership, structure and overview when planning their future during the transition from 18 to 18 plus.
